What information is collected?

The Center For Success collects information on certain areas of the Survey. The information collected may consist of information that you provide, such as names, mailing addresses, e-mail addresses, telephone and fax numbers. The Center For Success also collects certain information that you do not visibly enter, such as your IP address, browsing pattern on the Site, click stream, and the status of cookies placed on your computer by The Center For Success, as described below.

How may The Center For Success use my information?

The Center For Success uses your information to perform statistical analyses of the Survey data to be presented to our client.  You may elect to make your information and related Survey answers to be anonymous. The Center For Success does not provide personally identifiable information to third parties for commercial purposes such as the sale of products or services.

Are cookies and IP addresses used?

The Center For Success has implemented generally accepted standards of technology and operational security in order to protect information from loss, misuse, alteration, or destruction. Only authorized The Center For Success personnel are provided access to information, and these employees are required to treat this information as confidential. Despite these precautions, The Center For Success cannot guarantee that unauthorized persons will not obtain access to your information.
